Description:   If you use the APFRMF00.EXP file provided with the 11.4 CD, forms are trashed.
For example, field 10 Bottom stub start line is 800, field 11 Compressed print is blank, for forms CHK4I and CHK4L. If you view/change a form, the fields are not valid. If you look at the Reports (forms) list, the report is garbage.
